IT IS A DISCOUNT PLAN AVAILABLE TO PATIENTS WITHOUT INSURANCE TO HELP OFFSET THE COST OF DENTAL TREATMENT.

Planing Plan

Only $999/year ($1999 value)

4 quadrants of root planing and scaling
Perio maintenance treatments (3 per year)

20% off all other treatments (some exclusions may apply)*

*20% discount cannot be used for cosmetic treatments like Invisalign®, orthodontic treatment, veneers, cosmetic crowns, Botox, Juvederm, Zoom whitening, or Opalescence whitening.

Dugas Dental Savers Plan Guidelines

The annual fee of $249 or $499 is per person per 12 month period, starting on the date of registration
Eligible family members include spouses and dependent children up to age 21
The term “2 basic cleanings” does not apply to deep cleanings or periodontal scaling and root planing.
Membership fees must be paid at the time of enrollment and are not refundable, even if services are not used
Membership plan will automatically terminate at the end of 12 months if the enrollment is not renewed
If a patient uses the plan for care that extends beyond one-year (such as with Invisalign), the plan must be renewed annually and remain active for the entire duration of the treatment for the discount benefit to apply.

Exclusions and Limitations

This plan cannot be used with any other insurance or discount plan
This plan cannot be used to pay for any costs for dental care related to dental injuries that are covered under Workers’ Compensation
This plan cannot be used to pay for any costs for dental care related to hospitalization or hospital charges of any kind
This plan cannot be used to pay for any costs for dental care or injuries that are covered under medical or automobile insurance
If a patient uses Care Credit to pay for their dental treatment, a 10% administrative fee will apply
Dr. Dugas reserves the sole right, at his discretion, to cancel a member’s enrollment without refund and to refuse dental treatment to any patient or plan member
